City of Buffalo Licensed Third-Party Inspection Companies

All Licensed QEI Third-Party Elevator Inspection Companies,

In an effort to better insure that violations are corrected in a timely fashion please accept this correspondence as official notification from the City of Buffalo regarding important changes when submitting third party QEI elevator inspection reports.

Effective August 1, 2005 all inspection reports shall additionally include the City of Buffalo issued application number for each elevator/conveyance listed on a particular report.  These numbers can be found on the city issued certificates of operation issued to and posted by property owners.

The City of Buffalo Elevator Safety Review Board at its July 11, 2005 meeting recently discussed this issue and decide that every Elevator Maintenance Company listed should have access to and receive a copy of the semi-annual inspection report within 5 business days of its publication.  Therefore please continue to identify and list the name of the licensed Elevator Maintenance Company on all inspection reports.  Any changes regarding this information should be updated while conducting semi-annual inspections. Property owners shall approve or disapprove of any report being forwarded to an elevator company and their choice shall be posted on the inspection report.

If you have any questions regarding this matter please contact me ASAP at 851-4290, fax 851-4952 or e-mail: jschiavone@ch.ci.buffalo.ny.us

Respectfully,

Joseph L. Schiavone, QEIS #70
